Your first visit to our office establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your dentist. Patient Registration


We will not be mailing out paper registration forms as our office is considered paperless.  Once you arrive the day of your New Patient visit, you will be asked to complete the registration process on one of our computers at the front desk area. You will be asked to arrive at least 10-15 minutes early. If you do not feel comfortable using a computer, we can print out the registration forms.

Prior to your New Patient visit,  we may ask you to call your previous dental office and have your dental records (especially your X-rays) sent to us.  We are digital so if your previous office is digital, as well, they can be easily emailed and none of the quality is lost!  HIPAA requires the patient’s permission so it will be important to call as soon as possible to get this process started.

Being well-prepared for your appointment will ensure that Dr. Moreland has all the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling.
